Building Request Form

1. Reservations can also be made by contacting the Church Office at 616.647.1111.

2. Reservations will not be considered complete until the "Building Use Request Form" has been approved and returned to the person requesting use of the facility.

3. Outside groups using the facility must supply all materials for their gatherings. Office machines, office supplies, and consumable kitchen supplies are not available for use. Each group using the facilities is also responsible for removing all supplies and left over food and materials at the end of the event.

4. Any group bringing children must provide adequate supervision for their safety and to protect church property. Children may never be left unattended in any part of the church property. Please note,the Childrens Ministry area known as The Block, is not available to ourside groups.

5. Requested tables and chairs will be on carts in the room and must be returned to the carts after use. Room Set-up is available for a fee, otherwise we expect chairs and tables to be placed back on the cart at the end of your event, and therefore to be left in the manner in which is was found.

6. Nothing may be affixed to walls or furniture.

7. The following is prohibited on church property: smoking, smokeless tobacco, alcoholic beverages, and firearms.

8. The following is also prohibited in the church facility: roller blades, scooters, skateboards, ATV's and pets/animals (except those approved for a church program and those considered "service pets").

9. If you break something, you will be responsible for the cost of repair or replacement. If you discover something in need of attention or repair, please tell the church office.

10. Equipment (tables, chairs and other equipment) are not permitted to be used away from the church property.

11. All activities must be scheduled so that the event and clean up, by users, is completed by 10:00 pm on Sundays through Fridays and Saturdays by 7:00 pm.

12. Any activity that could be construed as gambling is prohibited in or on church property.

13. All rooms are to be left in the order in which they were found.

14. Access or use of areas of the facility not included in the Reservation Form is prohibited.

15. If the kitchen is used during an event, the Kitchen Policy is to be adhered to and a checklist completed at the conclusion of the event.

Kitchen Use Policies and Procedures

Kitchen use policies are intended to protect the church and the persons it serves. In order to serve Frontline Ministries well, and to maintain health and safety, compliance with these policies is MANDATORY for any person or group who uses the kitchen.

All kitchen use must be requested, approved by staff, and scheduled on the church master calendar. Kitchen reservations can be made by completing a Building Use Request form.

Policies

Dishes: All dishes must be washed, rinsed, and sanitized following the posted three-step method. All dishes must be put away after each use.

Countertops: Countertops must be cleaned and sanitized after each use.

Trash: All trash is to be placed in the trash can, which is to be covered at all times.

Floors: Floors must be swept and mopped after each use. Storing items on the floor is not allowed at any time.

Personal Items: Personal items or supplies must be removed from the kitchen at the end of use. Frontline is not responsible for lost or stolen items.

Equipment and Supplies: All Frontline Kitchen equipment and supplies must be left in the kitchen. Borrowing items is prohibited. A supply list is included in your packet and in the kitchen. Please note items that need replacement when you leave.

Food: Food left in the refrigerator must be in covered, approved containers with date opened and person or ministry name. Food left for more than a week will be disposed of by staff. Please dispose of leftover food at the end of your event. Dry goods need to be in large plastic, sealed containers, labeled with dates opened. No boxed or bagged food should be left on the shelf.

Cooking Appliances: Microwaves, Coffee Pots, and Toaster Ovens should be clean after each use.

Non-Commercial Appliances: Non-commercial appliances should be used with caution. (Freezers, roasters, toasters, waffle makers, griddles etc.) as they are generally a violation of Health codes due to fire risk.

Protective Clothing: All persons using the kitchen work areas must wear hair restraints or caps. Persons serving food or preparing food such as fresh fruits, vegetables, sandwiches, etc. must wear gloves and use appropriate utensils.
